Paper Forms

For preceptors who do not wish to engage with the app, students may print new, fillable PDF forms for their preceptors to sign. Note that these will still need to uploaded to the student profile in the app for Clinical Director Approval.

• Clinical Experience Forms

  • Observe Birth Log : for births attended by the student in a supportive role, this includes “doula” or supportive births outside your apprenticeship and may include any birth setting. The Witness may be any person who was there and witnessed your attendance, including the client. A midwife does not need to be present.

  • Required Home Birth Log: for logging minimum # of required homebirth experiences.

  • Required Planned Hospital Birth Log: for logging minimum # of required planned hospital birth experiences.

  • Initial Prenatal Exam Form: for logging initial prenatal exams, assist or primary phase

  • Prenatal Exam Form: for logging prenatal exam experiences, assist or primary phase.

  • Birth Experience Form: for logging birth experiences, assist or primary phase.

  • Newborn Exam Form: for logging newborn exam experiences, assist or primary phase.

  • Postpartum Exam Form: for logging postpartum exam experiences, assist or primary phase.

• Continuity of Care Forms

  • (Partial) Continuity of Care Form: for minimum tracking of primary births + 1 prenatal exam

  • Full Continuity of Care Form: for minimum tracking of primary birth + 5 prenatal exams across two trimesters, newborn exam, 2 postpartum exams

• Skills Logs

  • Prenatal Skills Log : for tracking progression of prenatal skills

  • Labor & Birth Skills Log : for tracking progression of labor & birth skills

  • Newborn Exam Skills Log : for tracking progression of newborn exam skills

  • Postpartum Skills Log : for tracking progression of postpartum skills

  • Additional Skills Log : for tracking additional required skills

• Biannual Evaluations

  • Biannual Student Assessment : for preceptors to fill out twice per year as a discussion point and check in on student’s continued fit for the preceptor’s practice.

  • Biannual Student Assessment of Preceptor : for students to fill out to evaluate their preceptor and their working relationship with their preceptor. This may be shared with preceptor or simply submitted directly to NMI, at the student’s discretion.

• Skills Requiring a Second Signature

  • Skills Requiring a Second Signature : NARM designates four essential skills as needing an additional preceptor,

    who is not your main preceptor, to also observe and sign off.
