I understand that the media recordings in the NMI media library are for loan to NMI students.

I agree to return media loans within two weeks of receiving them.

I understand that return postage is my responsibility.

I understand that the audio library is offered for historical and cultural interest. I must verify clinical information to be certain it is current and accurate.

I understand that I am responsible for the consideration and informed application of any information presented in the library materials. I am responsible for the choices I make in actual midwifery practice, and will not rely solely on the information provided in the brief presentation of library sources.

I agree to pay replacement costs of lost media loans: $100 for each DVD or Print Media title.
