We understand that plans can change from time to time. If you need to withdraw from the program, this form will notify the Administrative Office of your intent to withdraw.
Upon receipt of this form, the Administrative Office will review your current financial standing in the program, assess any refunds that may be applied to your account, and set your withdrawal date.
Once your withdrawal is processed you will no longer be an active student at NMI and any modules or clinical experiences submitted within the last 4 weeks will not be graded or approved. Once your withdrawal is processed, you will no longer be enrolled at NMI.
Withdrawing means that any ungraded modules and unreviewed clinical experiences (meaning your preceptor has signed but Clinical Director has not reviewed) will not be graded/reviewed. If you want to receive credit for submitted work, please wait to withdraw until it is returned to you. Also, please make sure your preceptor has signed your clinical experiences so the experiences qualify for NMI review.
If we have a backlog, meaning that you submitted modules or have signed clinical experiences in queue for beyond 8 weeks before submission of your intent to withdraw form, we will process these after your withdrawal, as this is outside our target timeframe.
